For-profit corporate ownership

Most nursing homes in these states are for-profit and most are held through layered legal entities: an operating company holding the licence, a property company holding the building, and often a management company between them. Each files separately with CMS, which is why a group of 60 buildings can appear in the raw data as 60 unrelated companies.

Nonprofit and government ownership

Nonprofit systems and public hospital districts hold a meaningful minority. In Texas specifically, county hospital districts hold the licence on hundreds of nursing facilities that private companies operate day to day under the state's Medicaid supplemental-payment programme — an ownership structure that is invisible unless the operator and the owner of record are separated.
