What is a nursing home chain?

There is no legal definition of a nursing home chain, which is why published lists of the largest operators disagree with each other so wildly.

Three different things people mean

A brand — facilities sharing a name. A chain home office — a CMS enrollment category for a central organisation providing services to multiple facilities. Or common control — one group of people or entities with a controlling interest in many buildings, regardless of branding.

Why the third definition is the useful one

Common control is what determines how a facility is actually run: staffing ratios, vendor contracts, capital decisions, and whether a problem in one building predicts problems in another. Branding does not.

How we define it here

A corporate family: entities whose facility portfolios overlap enough that they are plainly one business. Entities are grouped only when multiple structural signals — portfolio overlap, ownership relationships, governance evidence and entity roles — independently support the grouping; the specific production thresholds are proprietary. Lenders and mortgage holders are excluded. Of 10,280 owner organisations in our data, 412 resolve into multi-entity families.

Common questions

What is the largest nursing home chain in the US?

It depends entirely on the definition used — brand, chain home office, or common control. Rankings that count legal entities rather than facilities under common control produce very different answers.

What is a chain home office in CMS data?

An enrollment category for a central organisation that provides services to multiple certified facilities. It is one signal of a chain, but not the only one.
